Questionnaire For Ghost Witnesses

Here is a list of questions to ask someone who claims to have witnessed a ghost.

1. Person's info: name, age, age at time of encounter, sex, occupation, children
2. Details of encounter: location, time/date, weather, site type
3. Was the ghost visible?
4. Was the ghost in motion?
5. Did it move anything?
6. Did it make noise?
7. Did the ghost interact with any humans?
8. Were you scared by the ghost?
9. Do you think you are psychic?
10.Do you know any information about the location?
11. Did the ghost stay in one area?
12. Did it seem to know where it was? While in the presence of this ghost, did you experience any other phenomena?
13. Were there any animals present? If so what did they do?
14. Is this your first paranormal sighting?
15. Have you ever been accused of any hoax?
16. Did you play with a Quija Board for anything before the encounter?
17. Can you describe you mental state before and right after the encounter?
18. Have you ever been convicted of a crime?
19. Did the ghost leave behind anything?
20. Has anyone close to you passed lately?
21. Do you think you know who the ghost is?
22. Are there any children being affected by the encounter?


So this is my list so far. I have been reading books and writing stuff down. Thank you to W. Blackman for his book that gave me lots of help on this subject!

Don't forget this one:

23. Were you smoking/drinking/sniffing/inhaling any type of substance right before the ghost appeared??

Seriously though, you need to be careful with this one:

18. Have you ever been convicted of a crime?

It has a nasty potential of turning into a nasty situation. Because of how a conviction can affect one's life, this is often a sticky subject. If they are currently facing a conviction, they probably won't want to talk about it. If they have already paid for it, then they would probably just rather move on.

Personally, I wouldn't ask this one, but that's just me.



P.S. No, I don't feel this way because I have been, other than a couple of jail stints for fighting. I've just been involved in certain areas in which we had to be careful about what questions we asked.

18 I would ask just because you don't know whose house you are walking into. Would you want to go into a murderer's house I wouldn't. For me it is a question for my own safety. That is the only reason. That is not one I would ask at the home it would be a before hand question on the phone. When joining this group you are asked the same question. When you apply for a job you get asked that question. It is not to be rude or to bring it up again it is just for the safety of the group members. It also helps to find out the state of mind of the person. If they just spent 2 years in jail and they get out and that night they see a ghost, Me I would think it is their mind playing tricks on them. But do you see my reasoning behind that question. I have a child It might just be a mom thing! Sorry!

Oh...I kinda misunderstood what you were doing. I thought you were just taking down an, "I've saw a ghost statement." I didn't realize that you actually meant this as a questionnaire prior to an actual in-house investigation.

If that's the case, then yeah, you might want to know if the guy chopped up his neighbours a couple years back.

AH, yes -- that's awesome. Those are some note-worthy questions. I actually have a questionnaire up on GHoST's website for people who want us to investigate their homes for them if they believe they have a haunting. If it's alright with you, would you mind if I added some of those questions to that list to ask? I think the answers to those would really help us out!

Oh and yeah, good point about the crime question. I hadn't actually thought of asking a client that, but you make a good point there.

seriously. I understand the question, but it is on the line. Similar to "do you take any prescription medication" or " do you use drugs and alcohol" or "have you been diagnosed with a mental disorder"

i get the questions, yet as someone who has interviewed and worked with people with ALL of those types of problems, they can be very sensitive subjects. I also know that if it is an issue, the alot of people have no qualms about lying. That's why it is so crucial to understand not just what questions to ask, but also how to interpret people, hear what they are not saying, and how to properly interview. Having a list of questions works, but it is also very helpful to allow free form dialog, this way people will often disclose more than if they were just being interrogated.

That's a very good point. Simple psychology, really. If you are down-to-earth and friendly with people and talk to them like a neighbor may, they would most likely respond in a similar manor, rather than if you were to recite questions to them.

I think the way we have it set up though works just fine -- First, potential clients fill out our questionnaire, and then afterwards (after speaking with them through e-mail), we set up a date to meet with them. During this time, we can really follow-up with any questions we may have, yet it's more casual and relaxed. For better or worse, I don't believe Tony and I are very intimidating people lol.
